CFI - Choices for Independence at Pyareo Home

Choices for Independence (CFI) is a Medicaid waiver program run by New Hampshire DHHS (Department of Health and Human Services) and is designed to provide home-care in an assisted living facility as an alternative to a skilled nursing facility. Under the CFI program, individuals may live at Pyareo Home provided they meet both the financial and medical criteria of this program. Being a non-profit assisted living, Pyareo can only accept a limited amount of residents under the CFI Program. Interested seniors may join the waiting list for this program and be in line for the next available apartment by applying for residency. CFI Program at Pyareo Home include:

  • Health care services including assistance with ADL’s (Activities of Daily Living)

  • Medication management

  • 3 home-cooked meals per day, including snacks

  • Weekly consultations by our RN

  • Weekly vital sign assessments

  • Weekly housekeeping, linen change, and personal laundry completed by our aides

  • Planned recreational, and social activities

  • Local transportation — appointments, shopping, etc.

  • $70.00 monthly allowance (from SS)

  • Pharmacy delivery services

  • Heating, electricity, and Wi-Fi connection

  • Cable television

  • Lifeline personal response system telephone and call button

  • Individually controlled thermostats

  • Carpeting

  • Smoke detectors and sprinkler system

  • Maintenance

  • Community library, children’s play area, dining room, and sun room
